**PORTAL GUIDELINES**

Adhere to the following guidelines when composing your messages:

  • New concerns or anything not previously discussed with providers will require an appointment. 
  • Keep portal messages to 2-3 sentences. 
  • Nursing staff will review your message to ensure that your concerns are addressed accordingly within 24-48 hours. Subject to a $35 portal fee. 
  • Medical emergencies or any acute illness will not be addressed in the portal and will require an in-person visit. In case of an emergency, dial 911 immediately.

Set your password

This form lets you set a password for your patient portal account so you can access your medical information and send requests to your provider. For security purposes, we also ask you to provide some identifying information.
